Many employers offer vision plans in addition to medical coverage.  In most of these cases vision benefits are distributed through another provider such as VSP, EyeMed, Spectera, or Superior.  Most of these insurers do not issue member ID cards.

These plans generally cover an annual examination and either glasses or contacts.  In the case of glasses, a frame allowance goes towards the retail price of the frame, and a copay structure for the lenses and lens features you select.  


Coverage for contact lens fittings and contact lenses vary considerably.  The contact lens fitting, or professional fees associated with generating a contact prescription, may or may not be covered by your plan.  Some plans have a copay for the contact lens fitting, some will offer a discount, and others may cover it in full depending on the complexity of the fit.   Likewise, the coverage for the contact lens supply will also vary.  Some insurers cover a predetermined number of boxes of select brands while others provide an allowance towards the total price of the contacts.
